The Passport Authority allows 2 rescheduling or cancelling chances in a period of 12 months. This can be done free of cost. However, if you exceed this 2-times limit, your application will be totally cancelled and you have to resubmit a fresh passport application and pay the full fees.

Passports older than 15 years old require a fresh application using the DS-11 application form, not the DS-82 form. Lost or Stolen: You can apply for a fresh passport using the application form if your previous passport was stolen or you lost it.
